AFTER THE SEIZURE

A seizure may end before recovery does.

The period after a seizure is often called the postictal state. This is the time when the brain and body are recovering, and it can look very different from one person to another.

Some people recover quickly. Others may experience effects for much longer.

After a seizure, someone may have:

  • confusion

  • extreme tiredness

  • headache

  • muscle soreness

  • difficulty thinking clearly

  • memory problems

  • changes in mood or behaviour

  • fear, anxiety or irritability

  • temporary difficulty speaking or understanding

  • injuries from the seizure itself

Postictal effects can last from minutes to hours, and for some people certain symptoms can continue longer.

The person may also have little or no memory of what happened during parts of the seizure or immediately afterwards, even though the people around them remember it clearly. NICE specifically recognises confusion after an event and lack of memory for witnessed abnormal behaviour as features relevant when assessing suspected epileptic seizures.

Recovery is part of the seizure experience too.

Understanding what happens afterwards can help families, carers, friends and professionals respond more safely and with more patience.

WHY CAN SOMEONE BE CONFUSED AFTER A SEIZURE?

Confusion after a seizure is a recognised part of the postictal state for some people with epilepsy.

During this period, the brain has not yet returned fully to its usual level of function. Research describes postictal changes in alertness, memory, behaviour, language and responsiveness, and the severity and duration can vary widely between people and even between seizures in the same person.

Someone may:

  • not know where they are

  • struggle to recognise people immediately

  • have difficulty answering questions

  • repeat themselves

  • lose track of what has just happened

  • have trouble forming or retrieving memories

  • appear awake but still be confused

  • remember the time before the seizure but have a gap afterwards

Memory impairment is well documented in the postictal period, particularly after some focal seizures. Recovery of memory and other cognitive functions can continue even after the person appears outwardly awake and responsive.

The postictal state does not have one fixed duration. Reviews describe symptoms lasting from minutes to hours in many cases, while some cognitive, behavioural or psychiatric effects can persist longer.

This is why the person who had the seizure and the person who witnessed it can sometimes have very different memories of the same event.

For the person recovering, those minutes may be missing. For the person beside them, they happened in full.

WHY CAN SOMEONE BE EXHAUSTED AFTER A SEIZURE?

For some people, the seizure itself may last only a short time, but the exhaustion afterwards can last much longer.

The period after a seizure is called the postictal state. During this time, the brain is recovering from the seizure and gradually returning towards its usual level of function. Fatigue, drowsiness and changes in energy are recognised postictal symptoms. The duration varies considerably: some effects resolve within minutes or hours, while changes in energy, thinking or mood can sometimes continue for days.

IT IS MORE THAN JUST “BEING TIRED”

Researchers do not yet have one simple explanation for post-seizure exhaustion.

Older explanations sometimes described the brain's nerve cells as simply becoming “exhausted” after intense seizure activity. Current research suggests the postictal state is more complicated. Possible mechanisms include temporary changes in brain electrical activity, inhibitory signalling, neurotransmitters, ion balance and cerebral blood flow. Exactly how these processes produce different postictal symptoms is still being investigated.

For the person experiencing it, that recovery can feel like:

  • overwhelming sleepiness

  • very low energy

  • difficulty concentrating

  • slowed thinking or “brain fog”

  • physical heaviness

  • needing to sleep

  • struggling to return immediately to normal activities

Muscle soreness can add to that exhaustion after seizures involving strong or prolonged muscle contractions, particularly tonic-clonic seizures.

NOT EVERY SEIZURE HAS THE SAME AFTERMATH

Postictal recovery varies between people and can even vary from one seizure to another in the same person. The seizure type, duration and individual neurological circumstances can all influence what happens afterwards.

Exhaustion is also not limited to visibly convulsive seizures. Postictal fatigue and drowsiness can occur following focal seizures as well. Clinical descriptions of focal epilepsy include physical and mental fatigue, drowsiness, difficulty concentrating and confusion among possible postictal symptoms.

RECOVERY DOESN'T RUN TO A CLOCK

There is no universal amount of time in which someone “should” feel normal again.

ILAE reporting notes that postictal complications such as exhaustion, confusion, fear, headache, memory difficulties and behavioural changes may last an hour for some people and days for others.

That means somebody who still needs rest several hours after the visible seizure has ended is not necessarily “making a fuss” or simply lacking motivation.

The seizure may be over. The recovery may not be.
